CSR approved but not issued

when i approve Certificate Signing Request with command “kubectl certificate approve user”…then i see the CSR is approved but failed…it should be approved,Issued. I tried multiple times. Is i’m missing something?

Can you please refer to this link to create a CSR?
I don’t know what fields you have added to create a CSR “my-svc”.
